Forms (online)
To keep students safe, we need your cooperation. Some forms are online, and some of them need to be signed each year. Please go to https://myforms.vsb.bc.ca/ and log in using the student's PEN and birthdate. The Vancouver School District requires all the following forms to be signed for each student. Please indicate your Consent or No Consent. Some forms do not need to be signed again if you have signed them in the past.
▢ Neighbourhood Class Walk Consent
This one is really important for your student to do activities with the class! Sometimes, teachers decide to take their class to Douglas Park or another nearby location, as described in the form. If we do not have your answer, it means that we don't know whether you want your student to join the class when they go, or you want them to wait at the school. This one must be signed each year!
▢ Canada Anti-Spam Legislation Consent
The school sends newsletters to families, and the school's PAC sends their newsletters and event notices. Some of these could have information that could be considered commercial. We must ensure that we have your consent. This consent will be in effect indefinitely unless you choose to withdraw it.
▢ External Media Consent
This is about publication of your student's name, image, or grade by outside media. Please read this form carefully and indicate whether you consent or you do not consent. This consent is effective immediately and lasts until September 30 of the following year, unless it is withdrawn. Please read carefully about how to withdraw consent (if you consent at first) and what would happen.
▢ VSB Media Consent
This is about publication of photos, videos, images, and/or the name of your child by the Vancouver School District. Please read this form carefully and indicate whether you consent or you do not consent. This consent is effective immediately and lasts until September 30 of the following year, unless it is withdrawn. Please read carefully about how to withdraw consent (if you consent at first) and what would happen.
▢ Yearbook Media Consent
This is about publication of photos, videos, images, and/or the name of your child by the school to make the yearbook. Please read this form carefully and indicate whether you consent or you do not consent. This consent is effective immediately and lasts until September 30 of the following year, unless it is withdrawn. Please read carefully about how to withdraw consent (if you consent at first) and what would happen. At Emily Carr Elementary, the Parent Advisory Committee (PAC) makes the yearbook, and the specifics are described in the letter that was attached to the email from the office explaining these forms.
▢ G4-12 Student AUP
This is the Acceptable (Responsible) Use of Technology Policy, to make sure that students are aware of the expectations when they use technology at school. This should be showing only if your student is in grades 4 to 7. Please help your student to understand this and "sign" it.
▢ K-3 Student AUP
This is the Acceptable (Responsible) Use of Technology Policy, to make sure that students are aware of the expectations when they use technology at school. This should be showing only if your student is in Kindergarten to Grade 3. Please help your student to understand this and "sign" it.
▢ Adobe CC Acknowledgement of Use (for students)
Not all classes will use Adobe products. If your student's class does use Adobe, you will need to have this signed. Please help your student to understand this and "sign" it.
▢ Adobe CC Consent (for parents/guardians)
Not all classes will use Adobe products. If your student's class does use Adobe, you will need to have this signed. This is for a parent to sign. Consenting to this form gives your student permission to use Adobe CC, and this permission is granted for as long as your student remains with the Vancouver School District.
