Paper Forms

Two color paper forms are sent home on the first day of school.  Please return these forms to your homeroom teacher on or before Friday, September 6, 2024.  These include:

• Emergency Student(s) Reunification Authorization (for completion)
• Student Information Verification (for verification, and updates if needed)

Online Forms

Parents and students could access the online forms on VSB MyForms.  Submission is due on the Friday of the students' second week of school.  It requires the PEN and date of birth of the student to sign in.  Alternatively, one can sign in using VSB credentials, i.e., the VSB email and password.

The documents requiring a response of Yes or No include:

• External Media Consent (for completion by parents/guardians)
• Neighborhood Class Walk Informed Consent (for completion by parents/guardians)
• Request for CASL Consent (for completion by parents/guardians)
• Adobe Creative Cloud Consent (for completion by parents/guardians)
• Data and Image Consent for School Yearbook (for completion by parents/guardians)
• Acceptable Use of Technology Policy (for completion by students)
• Adobe Creative Cloud Acknowledgement of Use (for completion by students)
